I have a question about parental rights.?

I have a 10 month old son who I love more then anything. His father is in prison and has been incarcarted his whole life. He signed the affadvit of parentage but never had it notarized. Im not sure why but my question is because he is isnt on my sons birth certificate how do I go about terminating any rights whatsoever?
